Privacy Policy

UQ Inter-Professional Clinics (UQ Sport) respects the privacy of our patients and their families.

To protect your privacy, all Clinic staff are bound by the regulations of the Commonwealth Privacy Act 1988 and the National Privacy Principles amendments released in December 2001. These principles set standards for how we handle your personal information.

The information we collect

To provide you with the best possible care, it is important for you to be able to trust us and share with us all the information necessary to enable us to diagnose and treat you.

The clinic understands the sensitivity of these details and will only access and use personal information for the purpose of providing clinic services. When you attend a clinic, a record is made containing your name, address, date of birth, contact details and medical and referral information. Medicare and other Commonwealth benefit details are collected for funding purposes only.

Every time you attend new information is added to your record. Personal information is generally collected directly from you or via referral letters from third party sources. Therefore the accuracy of the information provided is dependent upon the source of the information, making regular updates necessary.

Why we need this information

The UQ Inter-Professional Clinics staff work together and share information so that we can give you the best possible care. By recording your treatment history, our staff can quickly identify which treatments will be most effective for you and also helps to reduce the likelihood of repeating tests that you have recently had.

How your information is protected

Your health information may be contained in paper records and may also be held electronically, or in other mediums depending on the tests and treatment you have had. All information is kept under strict security.

Only authorised Clinic staff will have access to your information and are each bound by a strict legal duty of confidentiality. It is an offence for our staff to give information out about you to anyone except under very limited circumstances.

Your access to information

UQ Inter-Professional Clinics support your right to access personal details, inspect records, take notes or make a hard copy of the information that we hold on file about you.

You also have the right to request access to your health record under the Freedom of Information Act 1992. If there is information that is incorrect or which you do not agree with, you also have the right to request that it be corrected.

Proof of identity will be required prior to granting access to files or for the purpose of providing new or updated information for client records.

Lodgement of complaints

If you believe that there has been a breach of privacy, you should contact the UQ Inter-Professional Clinics and provide proof of identity. A written confirmation of the complaint may be requested. Every endeavour will be made to handle complaints quickly.
