How to refer a client to UQ Inter-Professional Clinics |
Clinical Exercise Physiology
If your patient is under an Enhanced Primary Care Progarn (EPC) plan and you would like to refer them to UQ Inter-Professional Clinics please go straight to step 2.
Step 1
Print out UQ Inter-Professional Clinics Referral Form (PDF) and GP Consent Form (PDF). Please ensure patients contact phone number is on the referral form.
Step 2
Please make the referral out to UQ Inter-Professional Clinics
Step 3
Please fax referral to (07) 3346 6209 and our administration staff will contact the patient to arrange the initial consultation.
Department of Veteran Affairs
Department of Veterans Affairs gold card holders may be eligible for referral under a D904 Form (PDF). This allows UQ Inter-Professional Clinics to deliver the program at no cost to the patient. If you would like your patient to access this service please write the referral on a D904 or your practice letterhead.
Type II Diabetes group session
If you have a patient you wish to refer to the Type II diabetes Group education and Physical activity Program.
Step 1
If your patient is under a General Practitioners Management Plan (MBS Item 721) or General Practitioners Team Care Arrangement (MBS item 723) please fill out a Type II Diabetes Form Allied Health Group Services Medicare form (PDF).
Step 2
